STEPS TO A
SUCCESSFUL FIRST VISIT
Call Kwan Yin's front desk 503-701-8766 to speak with our front desk staff. They will help you select a time and date for your first office visit, collect insurance information, or discuss cash prices for the services you are seeking.
Complete the required form prior to your visit

You may come 30 minutes early to your scheduled appointment time to obtain paper copies.
Request any information that is pertinent from your health care team. Some common examples:

Blood work, stool or other labs done within 1 year.

Imaging (CT, ultrasounds, colonoscopies, etc.)- If done within 3 years.

Chart notes- Request the last 6 months if you have been regularly seeing a provider.
Cancellation Policy: Please be aware that we ask patients to give us 48-hour notice if they need to reschedule or cancel an appointment. Late cancellations or missed appointments will incur a fee, as we are unable to reschedule the appointment with another patient without sufficient notice.

Insurance: Dr. Sand currently accepts most insurance plans except Medicaid, Medicare, and Kaiser plans. As a courtesy, Kwan Yin Healing Arts Center will bill your insurance on your behalf. Insurance information given to Kwan-Yin Healing Arts Center by your insurance company is not a guarantee of payment. This may include information about covered treatments, copays, coinsurance, deductibles, and pre-authorizations.
It is your (the patient's) responsibility to read your own policy, know your coverage, and review the explanation of benefits statements regarding payments. Please remember your insurance policy is a contract between you and the insurance company, we cannot guarantee payment of your claims. It is ultimately your responsibility to pay for any balance not paid or covered by insurance. Any charges that are not covered by the given insurance company will be billed to you.

Below is a list of some procedures codes that may not be covered by your insurance, but may be necessary during the normal course of an examination to diagnose, evaluate and give you the care you need.
