Why Hormones Are Rarely as "Normal" as Your Labs Suggest
Standard hormone panels often miss the nuanced patterns that drive real symptoms. A TSH within reference range does not rule out thyroid dysfunction. An estradiol level reported as "normal" may still reflect an estrogen-to-progesterone ratio that contributes to PMS, heavy periods, or mood dysregulation. A testosterone result flagged as low-normal in PCOS may be secondary to insulin resistance — a root cause that standard labs rarely investigate alongside reproductive hormones.
Functional medicine approaches hormone evaluation differently. Rather than asking whether a value falls within a broad population reference range, we ask whether your hormonal milieu supports optimal function — and if not, what is driving the imbalance.
Dr. Rebecca Sand, ND brings post-graduate residency training in Women's Health, Fertility, and Integrative Medicine to hormone evaluation and treatment. Her approach integrates advanced functional lab interpretation, the gut-hormone axis, nutritional biochemistry, and evidence-based naturopathic therapies to address the root causes of hormonal imbalance, not just its downstream effects.
Signs You May Be Experiencing Hormonal Imbalance
Hormonal imbalances present differently depending on which hormones are affected and whether levels are relatively high or low. Common presentations include:
-
Irregular, heavy, or painful periods
-
PMS or PMDD symptoms
-
Acne, particularly along the jaw and chin
-
Unexplained weight gain or difficulty losing weight
-
Fatigue that doesn't resolve with rest
-
Brain fog and poor concentration
-
Hair thinning or loss
-
Low libido
-
Mood changes, anxiety, or depression
-
Hot flashes and night sweats
-
Sleep disruption
-
Cold intolerance or feeling cold when others are warm
-
Puffiness, water retention, or bloating tied to cycle
-
Difficulty conceiving
-
Postpartum mood and energy challenges
-
Skin dryness or changes in skin texture

Hormone Conditions We Specialize In
Polycystic Ovary Syndrome (PCOS)
PCOS is one of the most common endocrine disorders affecting women of reproductive age, yet it is frequently undertreated beyond oral contraceptives and metformin. We investigate the metabolic, inflammatory, and adrenal drivers of each patient's PCOS phenotype, and develop individualized protocols addressing insulin sensitivity, androgen regulation, cycle restoration, and long-term metabolic health.
Perimenopause & Menopause
The hormonal transition of perimenopause can span a decade and involves fluctuating — not simply declining — estrogen alongside progesterone insufficiency, adrenal changes, and thyroid shifts. We offer comprehensive evaluation and management of perimenopausal symptoms including vasomotor symptoms, sleep disruption, mood changes, cognitive changes, and metabolic shifts. Both bioidentical and non-hormonal approaches are considered based on individual risk profile and preference.
Thyroid Dysfunction
Thyroid evaluation in our practice goes beyond TSH. We assess free T3, free T4, reverse T3, and thyroid antibodies (TPO and TgAb) to identify subclinical dysfunction, Hashimoto's thyroiditis, and conversion impairment that standard testing misses. Treatment addresses the autoimmune and nutritional drivers of thyroid disease alongside direct thyroid support.
Endometriosis
While endometriosis requires a surgical diagnosis, naturopathic medicine offers meaningful support for symptom management, estrogen metabolism optimization, inflammation reduction, and fertility support. We work collaboratively alongside gynecology to provide integrative co-management.
Irregular & Painful Periods
Menstrual irregularity and dysmenorrhea are often dismissed as normal variants. They are not. We investigate the hormonal, nutritional, and inflammatory contributors to cycle dysfunction and work to restore regular, pain-free cycles through targeted functional medicine protocols.
Postpartum Health
The postpartum period involves profound hormonal shifts alongside nutritional depletion, sleep deprivation, and new physiological demands. We support postpartum recovery with attention to thyroid function, adrenal health, nutrient repletion, mood support, and breastfeeding-compatible interventions.

The Gut-Hormone Connection
One of the most underappreciated aspects of hormone balance is the role of gut health — specifically the estrobolome, a collection of gut microbiome genes responsible for metabolizing and excreting estrogen. When intestinal dysbiosis or reduced beta-glucuronidase activity impairs estrogen excretion, estrogen is reabsorbed into circulation rather than eliminated. This mechanism contributes to estrogen dominance, PMS severity, endometriosis progression, and risk of hormone-sensitive cancers.
Dr. Sand's training spans both digestive health and hormone medicine, making her practice particularly well-positioned to evaluate and treat the intersection of gut dysfunction and hormonal imbalance — an area where most practices specialize in one or the other.
What Functional Hormone Testing Looks Like
Beyond standard bloodwork, comprehensive hormone evaluation may include:
-
Full thyroid panel — TSH, free T3, free T4, reverse T3, TPO and TgAb antibodies
-
Comprehensive sex hormone panel — estradiol, progesterone, testosterone (total and free), SHBG, DHEA-S, LH, FSH
-
Fasting metabolic markers — insulin, glucose, HbA1c for PCOS and metabolic assessment
-
Adrenal and cortisol assessment — DUTCH test or serum/salivary cortisol panels
-
Nutrient status — vitamin D, B12, iron studies, magnesium, zinc, and others
-
Gut microbiome and estrobolome markers — including beta-glucuronidase activity
We interpret results in the context of your full clinical picture — symptoms, history, cycle patterns, and lifestyle — not as isolated numbers.
